Consent form

I desire to participate in activities conducted by German Dragons Singapore ('GDS'), including without limitation land and water training, races, and community activities (together 'Activities'). My participation may be in any capacity, such as paddler, drummer, steer, instructor, organizer, or volunteer.

In consideration of allowing me to participate in any of the Activities, I accept and agree as follows.

Risks and potential hazards (together, 'Risks') are inherent to and may arise from the Activities. Such Risks include or may result in personal injury (physical and/or mental) or death, and loss or damage of property. My participation in any Activities, and acceptance of these Risks (whether or not foreseeable or told to me), are entirely voluntary.

I am responsible for the security of my property. I store all property within GDS' premises, or any area allocated to me, at my own risk.

I acknowledge that any health or medical condition, previous or current injury, and medication I am presently taking may increase the Risks to myself and others. I accept that I am responsible for my safety and well-being, and

1.

Declare and confirm that I am in good health and fitness to participate in any of the Activities, and have not been advised against participating by a qualified medical person;

2.

Shall inform any Executive Community member or the Coach if I feel unwell before or during any of the Activities; and

3.

Consent to receiving assistance and treatment deemed necessary in the event of an injury, accident, or illness while participating in any Activities, and to bear the expenses incurred.

I shall adhere to these participation rules for each of the Activities:

1.

Comply with all reasonable instructions given to me;

2.

Not act in a dangerous or reckless manner, or such manner that may endanger myself or others; and

3.

Not participate in any Activities under the influence of drugs, other than the lawful use of medication.

GDS may refuse my participation in any or all Activities if I have not complied with this agreement or, in its opinion, my participation may adversely affect the conduct of an Activity or endanger myself or others.

I will not hold GDS responsible, or be entitled to a deduction of the fees paid or payable where:

1.

An Activity is suspended or canceled due to unfavorable weather, direction, or non-approval of authorities, pandemic, or circumstances beyond the reasonable control of GDS; or

2.

GDS refuses my participation in any Activities due to any cause in the participation rules above.

To the extent permitted by law, GDS and its officers, members, employees, contractors, volunteers, representatives, and agents shall not be liable for any costs, losses, or damages (including indirect, consequential, special, and punitive damages), and claims (including third party claims) arising out of or in connection with the Activities.

I consent to:

1.

the collection and processing of my personal information by GDS, and its sub-processors on a need-to-know basis, for the purposes of organizing and carrying out any Activities;

2.

images, photographs, and videos being taken of me during any Activities; and

3.

GDS reproducing, modifying, using, publishing, and distributing (together 'Use') my images, photographs, and videos for the purposes of analysis, reports, and the promotion of GDS, and the sport of dragon boat paddling.

The laws of Singapore (without regard to any conflict of laws principles) govern this agreement, and GDS and I submit to the exclusive jurisdiction of the Courts of Singapore.
